For all on line credit card registrations made after May 12, 2005, please print out your payment receipt and bring with you to the conference registration desk.
We encourage pre registration due to space and planning considerations --
Nevertheless, we will be accepting on site registration on May 15, 2005 at the conference.

Registrations are non refundable after April 20, 2005. Prior to that, cancelled registrations will only be 50% refundable.
Official CME Certificates are available only upon request directly from the Office of CME at the SUNY-Downstate Medical Center
Receipts of payment for conference registation will be provided by us.
However there is an additional $20 fee for an official CME certificate from SUNY-Downstate.
If you reguire an official CME certificate, then after the conference, please send your check or money order for $20 with a copy of your registration receipt to:
